Grievance Form

IHM does not tolerate inappropriate behavior from anyone involved in CYO Athletics. In our efforts to continue to provide an atmosphere of caring, growth and enjoyment, we appreciate your assistance.

All Grievances must be submitted to the IHM Athletic Board. CYO has been informed to let anyone from IHM who has a grievance know that they must submit this form to the IHM Athletic board. They will not discus any grievances until after they have been in contact with the IHM Athletic Director.

A grievance of any official may only be submitted by the head coach.

A grievance of any player, coach or spectator for any inappropriate behavior may be submitted by anyone at any time.
If there is a concern regarding a coach, his/her coaching, or playing time of any particular player for any reason, the IHM Athletic board will only respond once this form has been completed.

All grievances will be investigated and penalties may be imposed as warranted.
